Clinical Problems Associated with Transfusion Therapy


Abstract
It was cerebrated the tercentenarial anniversary of the transfusion of blood into human beings in 1967. In June of 1667, Dr. Denis and his associates transfused blood from a sheep into a fifteen-year old boy who had been bled many times as treatment for fever. In 1900, Landsteiner and his associates introduced the concept of blood grouping, and identified the major A, B and 0 groupings with recognition of the Rh group in 1937.
On terms of improvements in surgical techniques, scientific understandings of pathophysiology in diseases and anesthetic managements, the necessity of the transfusion was getting increased in both its amount and frequency. Many of the problems associated with transfusion therapy can be predicted by the biochemical impact of preservatives(ACD and CPD solutions) on stored blood and host reactions in recipients.
On these bases, the former topic included acid-base balance, coagulopathy, 2, 3-DPG(diphosphoglycerate) and the oxygen-dissociation curve, hyperpotassemia, hyperammonemia, citrate intoxication, hypothermia, red cell deformability, denaturation of protein, and vasoactive substances. The latter topic included various host reactions in recipients; allergic reaction, dissemination of diseases, bacterial contamination, hemolytic reaction, air embolism, circulatory overloading and pulmonary dysfunction due to infusion of debris in stored blood. Preservatives other than ACD solution and component therapy for transfusion were discussed also. Life span of red cell in stored blood, differences of red cell survival between ACD and CPD solution and deter iorations of plasma protein, platelets and leucocytes in stored blood were illustrated.
